Context
Given the growing needs for respite care, the IFQ has decided to meet the priority needs of many families with a developmentally disabled child, young or adult.

At the Marcelle and Jean Coutu Center, the residential care facility offers a maximum of fifteen (15) adpated placements.  All the space is now entirely accessible for handicapped persons. Living rooms are also provided for a better quality of life and the organization of many recreational activities. 

Goal
The goal of this facility is to provide respite care on weekends to carers of developmentally disabled persons. 


Definition of specialized respite care for weekends
By definition, specialized respite care is temporary and short term  residential services for a person living with a cognitive disability and having complex needs that require a specific specialized support. 

Clientele
The people concerned belong to two age groups.  Each weekend is for one age group at a time.

How to apply for the specialized respite for weekends?
Users of this service usually reside in Laval in their natural families and must register at the CRDI Normand-Laramée via the determined access procedure. They must contact a social worker of their  CLSC or their educator at the CRDI Normand-Laramée.

This model is also offered to non-Laval residents, in family-type facilities or intermediate homes, if placement opportunities are available.

TAKE NOTE: Additional costs may be charged if the user requires a staffing ratio greater than 1:3. Users must leave the facility before 8 p.m. on Saturday and before 7 p.m. on Sunday.

A specialized weekend respite is paid for by cheque made out to the Fondation IntégrACTION du Québec. We ask that parents provide the cheque at the latest on the Sunday evening when they come to pick up their child.

The Fondation IntégrACTION du Québec reserves the right to require a designated accompanying adult when a user is in need of a significant level of service. Additional costs can then be charged.

Reservation and Cancellation Procedures
As you already know, the goal of the specialized respite for the weekend is to provide rest to parents and other carers of a person with a cognitive disability or a disruptive development disorder. 

The demand for respite services is growing each year and the maximum number of places available for weekends is fifteen (15). When signing up for the first time to our specialized weekend respite services, application processing fees of $25 are charged (the fees are charged only once). The IntegrACTION Foundation of Quebec wishes to help as many families as possible by providing an opportunity to all families to access at least a few placements during a quarter. 

The IntegrACTION Foundation of Quebec informs families (parents of adults or children) that they have till the previous Wednesday of the respite to cancel a weekend. If the weekend is cancelled after this date, families concerned  will be charged $50. For some time now, some families cancel at the last minute their reservations.  The CMJC ends up with placements that remain unclaimed and it is inconvenient for another family to learn one day in advance that a respite placement is available. We ask the families to confirm us the desired weekends.  It is important for the Marcelle and Jean Coutu Centre that families honor their reservations.

Parents are asked to sign this form and return it to us as soon as possible. 

Reservation
All weekend respite reservations must be made via the Reservation form. Please honor reservations. Any cancellation must be filed at least one week in advance. It is essential that the parent call to plan the respite weekend.

The specialized respite care for weekends facility is open from Friday 4 p.m. to Sunday 7 p.m.  In case of lateness, the Center must be advised as soon as possible. You may be charged extra should you fail to arrive to or leave the facility on time.

Regulations for the specialized respite for weekends facility
- Personal file 
Parents are required to complete a personal file with a mandatory photo before the first stay of their child at the Center.  They must also sign the authorizations required by The IntegrACTION Foundation of Quebec. Parents are responsible for advising us of any change in the child's personal health record : intervention plan, support, medication, etc. The FIQ declines any liability in the event that such information is not forwarded in a timely manner

- Medication
Users are required to bring their medication in an adequately identified dosette identified with a photo of the user and his name. Parents must also ensure that the directions for use are accurate. The specialized respite care for weekends must be informed in writing of any change of the medication treatment. We recommend that users with severe allergies bring to the Center their « Épi-pen » kit. Users must carry their health insurance card. No nursing care services will be provided except following a special arrangement with a CLSC. Parents are responsible for advising the CMJC of any change in the child's personal record: intervention plan, support, medication, etc. The FIQ declines any liability in the event that such information is not forwarded.

- Allergies
At the specialized respite weekend all meals and snacks are included. Parents wishing to add anything to our menu need to know that the CMJC will refuse all products, foods and beverages that could have traces of nuts, peanuts and/or citrus fruits (lemons, clementines, kumquats, bergamots, limes, mandarins, oranges, grapefruit, pomelos, tangerines et les citrons) due to risks of a severe allergic reaction. We ask that people with known severe allergies to come to our facilities with their « Épi-pen ».

- Personal belongings
All clothing and personal belongings of users must be identified adequately. Users must ensure thay have a sufficient quantity of clothing for their stay at the respite facility. No washing of clothing is carried out unless clothing is dirtied. Users must supply at all times their incontinence panties and sanitary towels. Each person must have at all times a complete toiletry kit including all hygiene products. 

- Adapted Transit
Parents are required to reserve public or special private transportation to bring their child to the Center and return him home. The IFQ does not provide this service.
